We welcome our community to share in our blessings by using our facilities for activities and events.  Facility Request Forms are available below for those who would like to use the Church Building or Unity Hall.

      Request Forms must be filled out and submitted to the church secretary at least one week in advance.  Each form must have 3 signatures; the applicant, an active church-member whom will be present at the event, and the OCC Chairman, Vice Chairman, or their designee for approval.  The secretary can assist in securing the approval signature.  (Church staff are not authorized signers.)
